What is Form 1?
Form 1 – Assessment of Attendant Care Needs is completed by a qualified assessor (typically an Occupational Therapist). It evaluates how much assistance a person requires with daily living activities after an accident.

Level of Care
Determines how much assistance is needed for personal care, mobility, supervision, and daily tasks.
Monthly Benefit Amount
Establishes the dollar value of attendant care benefits that may be claimed from the insurer.
How it fits into the claim
The Form 1 is submitted to the insurer and reviewed as part of the accident benefits process. Approval depends on documentation, justification, and the insurer’s assessment of the claim.
