Many people in Ontario do not realize that they may be eligible for publicly funded home care services at no cost. Ontario Health atHome helps coordinate these services based on medical need.
What is Ontario Health atHome?
Ontario Health atHome is the provincial organization responsible for coordinating publicly funded home and community care services. It replaced the former Home and Community Care Support Services system and now manages access to care across Ontario.
What services can be provided?
Nursing & Medical Care
Includes wound care, medication support, and clinical services delivered at home.
Personal Support
Help with bathing, dressing, mobility, and basic daily living activities.
Therapy Services
Physiotherapy, occupational therapy, and other rehabilitation supports.
Care Coordination
A care coordinator assesses your needs and organizes services accordingly.
Is it really free?
Yes — for eligible individuals, these services are publicly funded. This means you do not pay out of pocket for approved services.
How do you apply?
You can apply directly or be referred by a hospital, doctor, or healthcare provider. In many cases, individuals or family members can self-refer.

Public Care
Funded by the government, structured by assessment, and may be limited in hours or flexibility.
Private Care
Flexible, immediate, and tailored support that can complement public services when more care is needed.
